The island that provides the setting for the TV series is in the Archipelago del Sulcis, on the south-eastern tip of Sardinia. This is the Island of San Pietro, whose clear waters, tall jagged cliffs, hidden beach coves and single residential centre (the village of Carloforte) have been made famous by the series. Many scenes were shot in the historical centre of Carloforte with its colourful houses and in the hospital, a second home for Doctor Pietro.
Pietro loves to run the trails of Capo Sandalo with his dog to the lighthouse on the cliff. Locations that have appeared on screen include the Spiaggia Grande della Calasetta on the Island of Sant'Antioco, the ancient tonnara of Portoscuso and the beaches of Chia, Cagliari and Villasimius.
